Plantar Fascitis 足底筋膜

Plantar fasciitis is irritation of the thick band of tissue under the foot, usually causing sharp heel pain with the first steps in the morning or after long standing. Physiotherapy helps by releasing tight tissues, correcting foot mechanics, and giving exercises to support the arch.

足底筋膜炎係腳板底一條粗厚筋膜發炎,多數會喺朝早落地第一步或者企耐咗之後感到腳踭刺痛。物理治療會幫你放鬆繃緊組織、改善足部受力情況,並提供針對性訓練去支撐足弓。

足底筋膜炎係腳板底由腳踭連到腳趾嘅筋膜長期受拉扯或退化,引起微細撕裂同發炎,最典型係腳踭痛。

常見原因
– 長時間企立、行路或跑步,尤其係硬地面
– 扁平足、高弓足或腳形結構問題
– 小腿肌肉過緊、阿基里斯腱繃緊
– 體重過重、穿著欠支撐性鞋款
– 突然增加運動量

症狀
– 朝早起身落床頭幾步刺痛最明顯
– 久坐再企起身會再痛一次
– 多數集中喺腳踭內側,按壓會特別痛
– 行耐、跑步或企耐會加劇

物理治療重點

急性期:減痛、減壓
• 儀器治療、冰敷減少發炎。
• 教導合適拉筋(小腿、足底)及臨時貼紮或鞋墊,分散腳踭壓力。

慢性期/恢復期:組織修復、改善足部力學
• 需要時使用衝擊波促進筋膜修復。
• 強化足弓及臀部肌肉,調整跑姿/行路姿勢,連同鞋款建議,從根源減少足底負荷。

Plantar fasciitis is irritation and micro-tearing of the plantar fascia, the thick band running from the heel to the toes, leading to characteristic heel pain.

Common causes
– Prolonged standing, walking, or running on hard surfaces
– Flat feet, high arches, or other foot structural issues
– Tight calf muscles and Achilles tendon
– Increased body weight or unsupportive footwear
– Sudden spike in training or activity volume

Symptoms
– Sharp pain in the heel with the first few steps in the morning
– Pain when standing after sitting for a while
– Tenderness at the inner part of the heel bone
– Worsening with long walks, running, or prolonged standing

Physiotherapy focus

Acute phase – pain relief and off-loading
• Modalities and ice to reduce inflammation and pain.
• Stretching for calf and plantar fascia, taping or temporary insoles to unload the heel.

Chronic / recovery phase – tissue healing and biomechanics
• Shockwave therapy when appropriate to stimulate healing of the fascia.
• Strengthening of the foot arch and hip muscles plus gait and running-form retraining, with footwear advice to reduce long-term strain.
