Shoulder Impingement 肩夾擊症
Shoulder impingement occurs when the tendons in the shoulder are pinched as you lift your arm. It often causes pain with reaching, lifting, or sleeping on that side. 肩夾擊症係當你舉高手隻手時,肩膊入面嘅筋腱被夾住,導致肩膊痛,舉手、拎嘢或者瞓覺側瞓都會唔舒服。

肩夾擊症係指當你舉高手隻手時,肩峰同上臂骨之間空間變窄,令旋轉肌腱同滑囊被夾住,引起疼痛同發炎。
常見原因
– 長期重複頭頂上方動作(例如打球、游水、搬貨)
– 圓肩寒背、頸前傾等不良姿勢
– 旋轉肌群無力或肌力失衡
– 肩關節退化、骨刺或舊傷
症狀
– 舉手、穿衣、梳頭或伸手入背時肩膊刺痛
– 夜晚瞓覺側瞓患側肩膊會痛醒
– 肩膊活動範圍下降,好似「卡住」
– 有時伴隨無力、沉重感
物理治療重點
急性期:舒緩痛楚、減少夾擊
• 冰敷、儀器(干擾波、超聲波)減少發炎。
• 溫和關節鬆動,改善肩峰下空間,配合輕微伸展。
慢性期/恢復期:重整肌力、改善姿勢
• 強化旋轉肌群、肩胛穩定肌及背肌,提升控制。
• 矯正頸胸姿勢,教授合適工作及運動動作,預防再次夾擊。
Shoulder impingement occurs when the space between the top of the arm bone and the acromion becomes narrowed, pinching the rotator cuff tendons and bursa during overhead movement.
Common causes
– Repetitive overhead activities (throwing, swimming, lifting, stocking shelves)
– Poor posture with rounded shoulders and forward head
– Weakness or imbalance of the rotator cuff and scapular muscles
– Degenerative changes, bony spurs, or previous shoulder injuries
Symptoms
– Sharp pain when lifting the arm, dressing, combing hair, or reaching behind the back
– Night pain, especially lying on the affected shoulder
– Feeling of catching or “block” in the shoulder with movement
– Possible weakness or heaviness in the arm
Physiotherapy focus
Acute phase – settle pain and reduce impingement
• Ice and modalities (e.g., electrotherapy, ultrasound) to reduce inflammation.
• Gentle joint mobilisations and stretches to open the subacromial space and ease movement.
Recovery phase – rebuild strength and posture
• Targeted strengthening of rotator cuff, scapular stabilisers, and upper back muscles.
• Postural correction and movement-pattern training for work and sport to prevent recurrence.
