De Quervain's Tenosynovitis 媽媽手
De Quervain’s tenosynovitis is inflammation of the tendons on the thumb side of the wrist. It causes pain when gripping, lifting, or twisting the wrist, and is common in new parents and people who do repetitive hand work. Physiotherapy aims to reduce irritation, improve tendon glide, and guide you on activity modification and support.
狄奎凡氏腱鞘炎(俗稱「媽媽手」)係手腕拇指側嘅筋腱發炎,拎嘢、扭手腕或者抱BB時會感到痛楚。物理治療會幫你減少腱鞘刺激、改善筋腱滑動,同時教你善用護托同調整動作,減低重複勞損。

狄奎凡氏腱鞘炎,俗稱「媽媽手」,係拇指側手腕嘅伸肌腱同腱鞘發炎,常見於抱 BB、煮飯、文職或需要重複扭轉手腕嘅人士。
常見原因
– 長時間抱小朋友、轉手腕、拎重物
– 手機打字、滑手機過多
– 反覆家務或文書工作
– 直接撞擊或扭傷手腕
症狀
– 手腕拇指側疼痛,向前臂放射
– 拇指出力、扭毛巾、開瓶時特別痛
– 握拳將手腕向小指方向屈曲會更痛(Finkelstein 測試)
– 局部腫脹、按壓敏感
物理治療重點
急性期:減少刺激、控制發炎
• 建議暫時避免重複扭轉及用力握緊動作。
• 以儀器治療、冰敷配合手法放鬆前臂肌肉。
• 必要時建議使用護托固定拇指及手腕。
恢復期:改善滑動、重建耐力
• 進行肌腱滑動運動,同時逐步加強前臂及手部肌力。
• 調整抱 BB、工作及使用手機姿勢,教導正確用力方式,減少復發。

De Quervain’s tenosynovitis is inflammation of the tendon sheath on the thumb side of the wrist, commonly seen in new parents, office workers, and anyone doing repetitive gripping and twisting.
Common causes
– Repeated lifting and holding of a baby or heavy objects
– Excessive texting and scrolling on smartphones
– Repetitive household or office tasks
– Direct strain or sprain to the wrist
Symptoms
– Pain at the thumb side of the wrist, sometimes radiating into the forearm
– Pain when gripping, twisting towels, opening jars, or lifting with the thumb up
– Increased pain when making a fist and bending the wrist towards the little finger (Finkelstein’s test)
– Local swelling and tenderness
Physiotherapy focus
Acute phase – reduce irritation and inflammation
• Activity modification to avoid repetitive gripping and twisting.
• Modalities and manual therapy to ease pain and relax forearm muscles.
• Splinting the thumb and wrist when needed for short-term protection.
Recovery phase – restore tendon glide and endurance
• Specific tendon-gliding exercises and progressive strengthening for forearm and hand.
• Education on lifting technique, phone and work ergonomics, and daily-task modification to prevent recurrence.
